Dear Cynthia , Because of the problems you mention we don't rely on flow for possible M7. On the rare cases including the Downs cases, we use immunoenzyme staining by APAAP for CD61 (Dako) on the BM smear or a cytospin. It works beautifully and it is easy to identify positive blasts and ignore the adherent platelets or giant platelets which can also be a problem. We would be interested to hear however of a reliable flow method.
