There are at least two free programs which convert ASCII columns of
numbers into FCS format. One is my A2FCS program, another is Joe
Trotters TEXT2FCS. These are listed in the free software catalog (see
below).

I can't help you with (1) or (3). However, you could take an existing
flow gating program and modify it. Source code is available for Tom
Bakker Schut's ANALYSE program which does quadrants, and you could
discuss source code with Joe Trotter for WinMDI.

A comprehensive, international catalog of free flow cytometry software is

A2FCS is listed under the MFI/FCS verification suite.
