unusual case

We had a bone marrow from a patient with a large percentage ( 60 % ) of
blasts in the peripheral blood ( 82 yr F ). There were two populations of
interest. An agranular CD45 dim population and an more granular CD45 dim
population. The phenotype of the agranular population ( 50% ) was CD5, CD7,
CD13, CD34, TdT positive and negative for all other T, B, Myeloid markers.
The more granular population was positive for CD7 CD33, CD13, HLA-DR, CD34,
CD11b ( dim ), TdT and MPO ( as before negative for all other markers ).

I'd be interested in the opinion of others. We are thinking AML with
maturation and with ectopic T marker expression.
