Another instance of cells that are apoptotic and Annexin V negative is reported in J. Histochem. Cytochem. 46, 895 (1998) Comparison of Annexin V and Calcein AM as early Vital Markers of Apoptosis in Adherent Cells by Confocal Laser Microscopy.  

The authors attempted to correlate Annexin V-FITC binding (using NIH 3T3 cells) with changes in morphology.  They found a subpopulation of cells that were apoptotic, but did not bind Annexin V.  To quote: "...some cells that clearly exhibited apoptotic changes on calcein visualization were annexin-negative."
