Here is the survey that Sue DeMaggio collected with the addition of those
who have written lately: (If you have any corrections, additions or

following information:
1) In which region the institution is located
"West, East, South, Midwest" in US,
International
2) If there is support for the facility other than rates
alone. (Subsidized) SUB
3) Name of the institution (I believe all are non-profit)
4) Special Arrangements are footnoted after the regional
section SP ARR
for example some have bulk rates, training rates etc.
5) THe appropriate charges for types of services offered
