I believe you are looking for the CDC's 1997 Revised Guidelines for
Performing CD4+ T-cell determinations in persons infected with HIV.  They
are published in the Morbidity and Mortality Weekly Report, Jan 10, 1997,
Volume 46, No.  RR-2 Supplement.
