Joanne,
It's hard to imagine any bug surviving the bleach and EtOH. In
addition to others' comments:
1. You didn't mention if you are using the necessary 0.2 micron sheath
filter.
2. Try decontaminating your sample cooling water. This water is in
such close proximity to the top of the sample tube, it is always a
suspect of ours if we have sterility problems.
