I am having difficulty classifying a case and would appreciate some help. The patient is a 76 yo female with a history of gastric lyphoma approximately 16 years ago (diffuse histiocytic by history) who now presents with an ovarian mass. The mass is composed of small monotonous lymphoid cells with round nuclei, coarse chromatin, small nucleoli and abundant clear cytoplasm with distinct cell borders. By CD45 versus RALS, the cells extend from the are where lymphocytes are found into the monocyte region. This is also seen on the Leucogate. The cells are positive for CD20, CD10, kappa (moderate), IgM (bright), CD25, CD11c (approximately 50%) and CD23 (approximately 30%). They are negative for CD5 and CD103.
