An outside hospital recently referred us a bone marrow aspirate for a
flow
cytometry leukemia/lymphoproliferative evaluation from a 19 year old with
anemia, fever and lymphocytosis.  Although the outside pathologist did
not see
anything in either the marrow or peripheral blood except occasional
variant
lymphs, I was unable to persuade him to contact his hematologist to ask
to
cancel this study.  Against my better judgement, we ran some basic
markers and
discovered that virtually all her granulocytes were CD33 negative.  This
result was replicated in 3 tubes.  CD13 was normal, and only a small
proportion of granulocytes were HLA-DR activated.

I am aware of the neutrophil adhesion defects associated with absence of
the
CD11/CD18 complex, but I've never encountered absence of CD33 outside of
leukemia or myelodysplastic syndromes, which this patient as far as I can
determine does not have.  Are there any inherited neutrophil disorders
that
lack CD33?  Any suggestions for further investigations?   Thanks
