NK cell lymphomas are a mixed bag. Many are T cell based. CD8 positivity is common. Some CD56+ malignancies are actually acute nonlymphoid leukemias. CD4 positivity would be very strange. Are you sure that the CD4 pattern is not dim staining? We see this commonly in AML. We have recently seen a young man with a very aggressive lymphoma/leukemia showing only CD56 and HLADR positivity. There were no T or B cell or myeloid antigens. Malignancy can do wierd things(eg "lineage infidelity").
