Dear Flowers, I'm really glad to receive all of your quick response to my question. Most of you would consider this case is a ALL with abberrant expression of CD13 and CD33. Some of you suggest me to do the TdT and MPO. Good suugestion. But we're not yet to evaluate the intraprep for detection of intracellular antigen. By the way, we have done the immunohistochemical detection of these two marker. As a result, my boss consider this is a biphenotypic acute leukaemia of AML with B cell phenotype. The points are that the immunohistochemical detection of MPO is strong position, coupled with positive Tdt staining by the same technique. The LCA is negative in this case. He also considered that the kappa light is dim positive by Flow. As a whole, the positivity in kappa light chain, CD19, CD10, CD34, CD13, CD33, HLA-Dr and negativity in CD20, CD14,CD2, CD3 and CD7 expression by flow cytometrical detection as well as the positive immunohistochemical staining of MPO and Tdt made the diagnosis.
