Hello, This discussion is really inspiring! Especially since yesterday I got on my table a case of 69 year old woman with enlarged spleen (18cm), no enlarged lymph nodes (periph. and by CT), normal blood values, PB and BM negative by morphology (no material for flow of BM or PB was taken). We have received fine needle aspiration of the spleen and by flow cytometry 66% of cells show the following phenotype: CD19+, CD20+, CD22+, kappa+, CD5 clearly positive, CD23-, FMC7+, CD11c-, CD103-, CD25 dim, CD10-. The case was sent from another town so I will not get cytogenetics. There were too few cells to freeze for any molecular studies. I enclose two photos of the smears from the FNA from the spleen. The cells were also positive for tartrate resistant acid phosphatase. I had to give my answer to the clinicians but I will be really grateful for your comments! Unfortunately I will have to wait 10 days to read them since I will be away abroad. Best wishes Anna
