Dear Dr. CASTILLO NAVARRETE, indeed, there seem to be 2 populations, negative and positive for CD34. CD34 is negative in most AML M3 cases. However, it is not a rare finding that a small subpopulation of younger cells exists at diagnosis or relapse of AML M3. Perhaps more common in M3v. Therefore, the answer should be Yes, it is 1 leukemia and it should be treated as M3 if other investigations (morphology, PML/RARa) show this subtype. HLADR negativity alone is not sufficient for making a conclusive diagnosis of M3. It should not be called biphenotypic or hybrid leukemia since these terms are occupied by cases with lymphoid/myeloid features. There may be more than one clone, though.
