It is not unusual to see low or absent CD45 expression in common ALL. We regularly see CD45 -ve or CD45 weak ALL and also cases expressing CD13 and/or CD33 though I couldn't comment on how many had exactly the phenotype you describe. We would report this case as common ALL with expression of myeloid antigens and wouldn't comment on the lack of CD45. I can't recall any cases from our department of CD45 negative AML.
