I have been using Reference Manager for several years, and I find it does
the kinds of things you are looking for quite well; i.e. allows importation
from Medline (CD ROM or on-line), customisation of key words, and
generation of bibliographies for papers or grant applications. I use the old
DOS version, but there is a windows version.
